Write a script to convert existing xrefs from IDs to paths and to add the target title as the link text.

Before:

xref:ssl-authentication-overview[] 

After:

xref:common/modules/con_ssl-authentication-overview.adoc#ssl-authentication-overview[SSL authentication overview]
