Description of problem:

After filling data into data connection creation modal and close it (either by clicking on "cancel" or "add" or "x" button) the fields do not get cleared. Next time user clicks on " Add data connection" to create a new one, the fields show the old values.

It looks like happening in the scope of the same project. If user changes project, the fields get cleared

Prerequisites (if any, like setup, operators/versions):

Create a DS Project

Steps to Reproduce

1. enter your DS project
2. click on "Add data connection" button
3. insert the requested fields
4. close the modal 
5. click once again on "Add data connection" button

Actual results:

fields do not get cleared after modal is closed

Expected results:

fields get cleared after modal is closed

Reproducibility (Always/Intermittent/Only Once):

Always when opening the modal more times in the same project
