Problem: Users need a UI to be able to subscribe to receive notifications for different events related to products and incidents.

Acceptance Criteria: A page in the WebRCA UI that permits users/teams to sign up for email/slack notifications for incident product declarations and incident lifecycle events.

Additional Information: This could be a separate page in the UI like the teams and products tab are . Also users could possibly subscribe in the notifications or details page of the incident.
