      Issue description

      ePub rendering of ADOC incorrect because of missing subs=+macros

      The rendered PDF and HTML do not have issues, but viewing the ePub in RHTB or Calibre or anything else that can render an ePub file will show the ADOC code and not display the content correctly.

      Steps to reproduce:

      Go to source boxes with keyboard or button commands and look at the rendered output. This is easier to see in the Chapter 5, Section 5 section where it shows how to generate a GPG key.

      Workaround:

      Use PDF
