Description

As a user, I want to edit D/DC using a form instead of having to edit the YAML.

Acceptance Criteria

1. Create a form for editing D/DC by using existing Formik fields (or by creating new Formik fields if we need to add fields which doesn't already exists).

Additional Details:

Form fields are tbd.
3.x form fields are being evaluated to find out which fields are still relevant in 4.x.
This Spike will provide these details.
