Alerts are shown for all failed/successful plans. Testing should cover that once a migration plan fails, then an alert will be firing.

Alerts are firing until the source plan is deleted. 

Alerts contain information about migration mode (Warm, Cold), source provider, plan name and plan phase where migration failed.

New metric "mtv_plan_alert_status" was added in order to allow for simpler alert handling.
