In MTV 2.9.1, the admin is not presented with an option to create a network map using form view. Instead, there is no selection available. When selecting Create NetworkMap button, it opens the yaml view.

1. switch to admin view
2. in Virtualization main menu, select Network maps submenu item
3. check the Create NetworkMap button

a selection triangle for options  is available and when clicking, one gets presented with "Create with form" or "Create with YAML" to click on
