Steps to Reproduce:

STR:
1) Create rules with effort > 0 and with issue-display-mode="details-only".
2) Generate the report with apps that trigger it.
3) The story points (effort) in "Application List" do not match the "Report Index" of the individual apps.
4) The incident count on the application list will not match the counts on the migration-issues page

Example: WindupArchitectureDuplicateTest

It doesn't make much sense to have rules set to details-only and also with an effort value (details-only are only intended for info classifications). We should handle this and print a warning to the log and to the report.
