Backend request is implemented by the lua module backend_client.lua. Hence, opentelemetry_propagate directive does not take any effect. The headers need to be added in that module.

According to opentelemetry instrumentation, some nginx variables are set by the instrumentation and they can be used to propagate them to backend.
