Include "Approval required" setting on the Application Plan object

Some contracts (Application Plans) require a special agreement between the Buyer & Provider before the subscription is made. Currently this only works when creating a new Application and subscribing to such a Plan but in the case the Buyer is changing plan then this is not possible. The setting: "Request to change plan only" is on the Service level and therefore doesn't fit the use-case as a blanket setting.
