Document APICAST_REGISTRY_URL env var in INSTALL.md

      Since https://issues.jboss.org/browse/THREESCALE-2327, system won't crash in case APICAST_REGISTRY_URL is omitted. However, it's not clear for developers how to enable policies and get them fetched from a running APIcast.

      So this issue is to add a brief explanation somewhere - not in the INSTALL.md - maybe a separate "integrating with apicast" document.
