[documentation] remove instructions for self-managed gateway deployment

      after executing all the commands to setup the OS:
      https://access.redhat.com/documentation/en-us/red_hat_3scale/2.saas/html/deployment_options/apicast-self-managed#step_1_install_openresty_and_dependencies
      and

      sudo luarocks make apicast/gateway/*.rockspec --tree /usr/local/openresty/luajit

      I tried to execute

      THREESCALE_PORTAL_ENDPOINT=https://*token@lmf-admin.3scale.net APICAST_RESPONSE_CODES=true APICAST_SERVICES_LIST=2555417758343 bin/apicast

      but receive the following error:
