Rename instance_mesurements.instance_id to host_id

Tech Debt:
The column on the instance measurements table named 'instance_id' is actually a reference to the host table 'id' field. To avoid confusion we should rename the field on instance_measurements to host_id.

Notes:
Make sure tally_instance_view is updated with new name.
