      Description of problem:

When you attempting to create a NodeHealthCheck using the webui, we experience an error:

Something wrong happened Cannot read properties of undefined (reading 'h1')

This occurs on Chrome and firefox.

Version-Release number of selected component (if applicable):

      OCP 4.19.4
      CNV 4.19.1
      node-healthcheck-operator.v0.9.0

      How reproducible:

Navigate to Compute -> NodeHealthChecks -> Create NodeHealthCheck in the Web UI

The issue is also seen if you go to Installed Operators -> Node Health Check Operator -> "Create NodeHealthCheck"

      The customer states:

Sometimes the selection box for Form view vs YAML view will appear, and I can select YAML view. But returning to Form View brings back the "Something wrong happened" error. At other times I will not get the option to choose Form or YAML view, and only the error appears.

      Additional info:

We have a customer who has seen this in a support case. I was unable to replicate on a fresh install in the lab.
