      Description of the problem:

      Checkbox Use escalating remediations selection is not visible

      How reproducible:

      100%

      Steps to reproduce:

      1. Login to web-console

      2. Go Compute/NodeHealthChecks - > Create NodeHealthCheck

      3. Choose `Use escalating remediations`

      Actual results:

      Can't see checkbox selected

      Can't return back to single remediation

      Expected result:

      Checkbox marked as selected/unselected
