Description of the problem:

Replacing other remediation template with SNR template during editing action

Steps to reproduce:

1. Create NHC with Other remediator

2. Go to Edit NodeHealthCheck page

Actual result:

Edit NodeHealthCheck page appears with SNR as remediator

Expected result:

Edit NodeHealthCheck page appears with Other remediator that was specified during creation

It happens every time when Form view choose
