use lv hash tables when parsing VG metadata

      In the VG text metadata parsing functions, lv and pv hash tables are used to look up LVs when creating the connections between various parts of the VG struct, but these hash tables were not being used in some cases. Using them in place of linear list searches for LVs speeds up metadata parsing significantly.
