Opening new chat session muddles history with empty entries

      A number of blank/empty entries are populated into the Chat history when a user repeatedly creates New chats. This also just happens whenever a new chat is created. If there is no content to be added, the chat history should not be updated.

      Repro Steps

      1. Login and open the chat assistant
      2. Expand the left-hand menu and click "New chat"
      3. Keep creating a few new chats
      4. Observe the Chat history, it is full of blank entries
