"rhc_connections" and "source_rhc_connections" missing from the initial Go migration

The initial migration of the Go application didn't include the "rhc_connections" and "source_rhc_connections" tables. This happened because the PR stayed open while those two tables were being added to the Rails app via ActiveRecord, and we forgot to include them.
