Description of problem:

If you create a "BRL Action" column and subsequently update the column definition the action is duplicated in the decision table: (1) the original column, (2) the edited column. This leads to unpredicted behaviour.

Steps to Reproduce:
1. Create a Guided Decision Table
2. Add a "BRL Action" column. Click OK
3. Update the "BRL Action" column. Click OK.

Actual results:

The "BRL Action" column is duplicated; once for the original (unedited column) and twice for the edited column.

Expected results:

The "BRL Action" column should not be duplicated.
