Formating issues in doc book pdf and html

Both html and pdf should be reviewed for formating issues.
  - All tables need to be reviewed for formatting issues
      (e.g. Table 8.2.  in the PDF)
  - All example code blocks need to be reviewed for formatting issues.
  - There are some paragraph issues as well.
