Description of problem:

In Review and finish tab of Create Robot Account wizard,  there is no table head for teams and repositories table.

Steps to Reproduce:
1. Create a robot account in new UI in Create Robot Account wizard
2. In Add To Team tab, choose some existing teams
3. In Add To repository tab,  choose some existing repositories.
4. Go to Review and Finish tab,  check the Teams table and Repositories table display status

Actual results:

The Teams table and Repositories table just have table body to show items, but without table head to describe the usage of every column.

Expected results:

Every table should have a head row to describe the usage of every column. Just like what the organization table does.
