Quay upgrade docs are not clear and needs clean up

The whole upgrade docs[1] needs to be re-written since its very confusing for users and there no clear steps on upgrade from 3.3.x to 3.4.x for standalone Quay/Clair

We have many redundant sections like 

Chapter 2. Red Hat Quay v3.y.z to v3.4.1 upgrade

Then again we have a section 

Chapter 6. Running the upgrade to Red Hat Quay v3.4.1

Both these section needs to be rewritten and we need to split them to capture upgrade procedure from major versions

• 2.9.x to 3.0.x 
• 3.0.x to 3.2.1
• 3.2.1 to 3.3.0
• 3.3.x to 3.4.x

Also upgrade steps which are captured for operator[2]  3.3.x to 3.4.x needs to be moved here
