Description of problem:

Unable to install `Nodejs Ex K v0.2.1` helm chart. The form does not any error when the `Install` button is clicked, but error is logged in the browser's console

Prerequisites (if any, like setup, operators/versions):

Steps to Reproduce

1. Goto `+Add`
2. Click on `Helm Chart` card
3.  Select `Nodejs Ex K v0.2.1`
4. Click on Install

Actual results:

• No error is being displayed nor is the form submitted
• error `An invalid form control with name='' is not focusable.` is logged in the browser's console

Expected results:

the helm chart should be installed successfully
