Description of problem:
Disconnected installation fails because the index image iib:<tag> used in mirroring does not guarantee inclusion of bundle and operator image digests. This leads to: - Bundle unpack timeouts: missing windows-machine-config-operator-bundle - Operator pod image pull failures: missing windows-machine-config-rhel9-operator - Subscription gets stuck without any CSV/InstallPlan being created

Steps to Reproduce:
1. Mirror the above index using oc adm catalog mirror or skopeo.
2. Observe only catalog.json is present.
3. Subscribe to WMCO — no InstallPlan or CSV is created.
4. Bundle unpack job fails to pull from registry.stage.redhat.io.
