The examples of Skipping upgrades and Replacing multiple Operators need to be updated. 

The currently recommended approach is to use a file-based catalog format. Upgrade information is defined in the channel rather than in the CSV. Section "Skipping upgrades" and "Replacing multiple Operators" need to be updated.
