Description of problem: A customer rolled out multipathing via machineconfig as a day 2 operation as outlined by documentation. Since then, the LSO is failing to make a new volume on one node.

Uncertain if this is an issue with the LSO, mpath in general, or a unique configuration but nothing in the data seems to provide a clear answer. Additionally, I have encountered cases with issues stemming from enabling mpath via mc rather than at the time of install.

For the other cases, rebuilding and replacing the node, and enabling mpath at that time resolved the issue. But investigation is needed to determine if this is a continuing problem with enabling mpath in this way, and what circumstances present conditions for it to occur.

Linked case has must-gather and sosreports from the nodes, node 3 is the one encountering issues.
