How reproducible: Always{code:none}

    

    1. Start with any cluster install of 4.16
    2. Install OCP virt either via the console or the CLI
    3. Go to Storage -> Persistent Volume Claims
    4. Click Create Persistent Volume Claim on the top right, and select with Data Upload Form.  
    5. In the Form, select Browse on the Data Upload box, select an image, and hit okay
    6. Or Open a file browser, select and image and drag it onto the same upload box 
    

  
    Nothing happens. Nothing shows in the UI.
    Even if I fill out the rest of the form and press upload, it said "File input is missing"
    It does work with Virtualization -> Bootable Volumes -> Add Volume -> From Form

    

    I expect the file to populate in the input field and the upload to work.
