Description of problem:

IHAC who reported that when trying to create an application using the "Import from Git" workflow, the "Create" button at the very bottom of the form stays inactive.

Seems similar to the bug https://issues.redhat.com/browse/OCPBUGS-11989 but it has been fixed in 4.11 , 4.12 and 4.13 but customer is still facing this issue in 4.14 version.

How reproducible:

Steps to Reproduce:

    1. In developer console click on +Add
    2. Go to Git Repository and click on it
    3. Click on `import from Git` and add `Git Repo URL` and other details.
    4. check if  "Create" button at the bottom of the form is inactive and cannot be clicked.
     

Actual results:

    "Create" button at the bottom of the form is inactive and cannot be clicked.

Expected results:

    "Create" button at the bottom of the form should be active and can be clicked.
