Cluster ownership transfer modal to include 3 fields in the UI: Username, Org ID and Account number (EBS account ID).

Acceptance Criteria:

• Modal with transfer modal shall appear
• 3 fields will be available for recipient username, recipient account, and recipient org (currently transferring to another org is not complete on the backend)
• The 3 fields will have validation.  Currently they are each 64 max characters and only allow a-Z and 0-9 will be allowed for org and account. Username will have no character restrictions. 
• Update: all three fields are required now even though they are not necessarily needed.  For consistency, we shall require them for all transfers. 
• On submit, an error will show if there is a problem or a popup notification will show in the upper right if successful.
