Not sure if this is intentional or not, but if we want users to enter the Provider Resource Name before they select a provider type, place the Provider Resource Name field above the types.

Right now, if you work from top to bottom, you click the type and the Type opens immediately so you can't fill in the provider name in Create Provider.

Suggestions: Either move the Provider Resource Name field to be above the types or eliminate it from the Create Provider screen since you have to enter it in each Type.
