[UI] Source provider field not getting populated on plan wizard

      Description of problem:

      After clicking "Create migration plan" on a provider page,  "Source provider" on plan wizard does not get populated automatically.

      Steps to Reproduce:

      1. Click "Create migration plan" on an existing provider
2. "Source provider" field on plan wizard does not get populated automatically.

      "Source provider" field on plan wizard does not get populated automatically.

      Expected results:

      "Source provider" field on plan wizard gets populated automatically.
