When entering invalid vddk init image value within the vSphere create provider form, and then clicking "skip vddk" for ignoring this field:
The field value is reset/disabled but the validation still occurs and blocks the user from creating the provider.

1.Click create vSphere provider
2.Set provider name, url and an invalid vddk value
3.click the 'skip vddk' checkbox for avoid using vddk

The vddk field is disabled and cleaned but form validation is failed on invalid vddk and therefore the create provider button is disabled

once 'skip vddk' checkbox  is checked, vddk field value should be ignored
