Description of problem:

Unable to create OSP provider with authentication type - token and username, the "Create Provider" button is disabled with unexpected message: "missing required fields [userID, projectID]"

1) Try to create OSP provider with authentiction type - tocken and username, input all the informations in web page

2) Check the "Create Provider" button is disabled with unexpected message: "missing required fields [userID, projectID]"
