While viewing a questionnaire , the view has two links that take one back to the 'Assessment questionnaires' page

Steps to Reproduce:

1. From the questionnaire kebab menu, click 'View'

Actual results:

Observe that questions get listed . This page also has two links that take one back to the
Assessment questionnaires page:
1) First link in the breadcrumb shows 'Assessment'; Clicking on this link takes one to the Assessment questionnaires page
2) There is also a 'Back to questionnaire' link that takes one back to the Assessment questionnaires page

Expected results:

One of the two links should be removed.
