Steps to Reproduce:

1. Open an assessment questionnaire
2. Close the questionnaire form or click "Cancel"

Actual results:

A confirmation message is displayed: "Are you sure you want to close the assessment? Any unsaved changes will be lost." while the "Save as draft" button is disabled
