URL handler creation can result in stack overflow

Since URLs may be required to locate URL stream handlers, a stack overflow situation can result. Fix this by detecting reentrancy on a per-protocol basis and bail out if it is detected, as it is not permissible to require a URL handler to exist in order to load the module which provides that URL handler.
