[KSW] - Persistence: add workflow id and version as the constraint keys when persisting the flow state

      More than one version of a workflow definition can be deployed in a given environment, having only the `version` changed. In such situations, we can have problems in the database if shared among the instances.

      Ideally, we would use the id and version of a given workflow definition as the composite key for persistence.

      All the persistence add-ons supported on DP1 must support this feature.
