ERROR: duplicate key value violates unique constraint "milestoneentity_pkey"
To reproduce simply run the AdHocFragments process twice from integration-tests-quarkus-processes, that will trigger a milestone event with the same id.

milestones=[MilestoneEventBody{name='AdHoc Milestone', status='COMPLETED', id='_731518E6-88A2-4CCC-A2C0-97C45189A7F3'}
where the id is actually the nodeDefinitionId.
