Include the cluster name in the GMS print_local_addr logging

With the shared transport, the old favorite print_local_addr logging is less helpful since multiple channels have the same Address. Helpful would be something like:

03:20:43,099 INFO [STDOUT]
-------------------------------------------------------------------------------------
GMS: address is 10.16.93.85:57379 cluster name is XYZCluster
-------------------------------------------------------------------------------------

Actually, that's helpful anytime a system has multiple channels, even w/o the shared transport.
