This patch adds support for having a default value assigned to existing rows when adding a column to a table - just like the SQL statement.

The patch adds an optional "default-value" element to "cmp-field" and "property". Also two new elements "add-column-default-template" and "add-column-default-supported" are added to "type-mapping".

The documentation patch describes the changes – only figure 11.4 and figure 11.7 needs to be updated to include the new elements.

Patch is against Branch_4_0 for code and HEAD for documentation.

Let me know if a port to HEAD is needed for the code.
