Add a global refresh icon to the header

Add an icon to refresh the current application view. The icon should be placed left to the expert/normal mode switcher in the header. Similar to the other icons, the refresh icon will show up if the presenter implements a specific interface:

Some applications already have a manual refresh icon as part of their UI. Remove these icons in favour of the global refresh icon in the header.
