      Update Integration test creation form to support Git repos

      Need to add following fields
      1. Must provide user the option to suppy Git Resolver as well as bundle resolver
      2. GitHub URL input and subtext, with URL validation
      3. Git options input like revision and path.
      4. Add relevant error message as per the doc

      Any other field added as per the doc
