Marketplace subscription billing model for Add-ons

      The add-on installation screen should allow customers to see what subscription billing model and the cloud provider billing account ID for the add-on that they are installing. This is similar to how we surface this information on the OSD cluster provisioning screen. Current options for the OSD provisioning screen are "Standard" and "Marketplace" (assumed to be RHM) and we need to expand this list to accommodate the various additional marketplaces that we will introduce as part of project Stratosphere.

      Additionally, if a customer has purchased the add-on from different sales channels (RH or the various marketplaces), they should be able to select the billing model, marketplace, or cloud provider billing account ID for their add-on during installation.
