M2K waiting on QA notifications pop-up doesn't pop-up

Steps to reproduce:

1. Deploy orchestrator 

2. Deploy workflows

3. Run m2k workflow with valid parameters

4. wait for 'waiting on QA' pop-up

5. observe it never shows up, see video for details

Notification pop-up never show up
