1) Create a BKM or a Decision node with a Function
2) Create a table inside of the Function
3) Add another output column
4) Table's header should have only one row

When a decision table is included in a function and more than one output column is present, the header row is duplicate

This issue was spotted also in ~~KOGITO-2410~~. So the fix should works also for Drawing 1 (86).dmn
