XLS Decision table: Add support for "otherwise" values in Condition Column cells

Add support for "otherwise" cell values in Condition Columns. E.G. "not in (x, y, z, ...)" where x, y and z are explicit literal values in the column. "not in" supports the equality operator, ideally other operators should also be supported.
