Remove duplicate partitions in TaskSyncContext.

After rebalance events, there are sometimes duplicate change stream partitions from dead tasks in the TaskSyncContext. Duplicate partitions can cause watermark stuck and other weird issues after rebalance events or connector restarts. We should detect the potential dead task and remove the duplicate partitions.
