Let's create a template for pull requests against the different Debezium repos. It should be as light-weight as possible, probably containing not much more than that:

• A link the contribution guide
• Perhaps point out key things to consider (commit message format, formatter must have ran)
• A basic check box list: tests, documentation updated, change log notes provided (if applicable), license checked (after major dependency updates)
