Scope mined DDL events to include/exclude lists if provided

Currently the LogMiner implementation does not apply table include/exclude predicates against DDL operations and this means that we are often parsing DDL events for objects that aren't of importance to the connector.

We should effectively move operation type 5 to the same query predicate where we have insert/update/deletes.
